How Smoking Impacts Bone Health

Smoking and Bone Density: The Silent Breakdown

Smoking weakens your bones in two key ways:

1. Direct damage to bone-building cells

Smoking is toxic to osteoblasts—the cells that build bone. It also restricts blood flow, which reduces the delivery of nutrients needed for bone repair and strength. This means that bones become weaker and less dense over time.

2. Disruption of hormone and nutrient balance

Smoking lowers levels of vitamin D, oestrogen, and impairs calcium absorption—all of which are essential for maintaining healthy bones. It also disrupts the balance of parathyroid hormone (PTH) and vitamin D, a crucial relationship for regulating calcium and phosphate in the body.

These effects combined lead to:

  • Lower bone mineral density (BMD)

  • Accelerated bone loss

  • Increased risk of fractures, particularly in the hips and spine

In fact, studies have shown that current smokers are up to 74% more likely to experience spinal fractures compared to non-smokers.
